Dr. Pagnani is trained in prescribing medications such as Antabuse (Disulfiram) and Naltrexone (oral or Long Acting Injectable “Vivitrol”) for Alcohol Use Disorder and Alcoholism.
Dr. Pagnani is licensed to prescribe buprenorphine (Suboxone) and depending on patient location and state rules and regulations he may also be able to prescribe long-acting injectables such as Vivitrol for opiate dependence and Sublocade (Long Acting Buprenorphine). Full psychiatric evaluations are required for addiction services, as there are often co-morbid conditions (dual diagnosis) affecting cravings and substance use (such as a mood or anxiety disorders, which may warrant additional treatment).
Psychiatric Medication Dependence and Withdrawal: “Medication Tapering and De-Prescribing.”
When appropriate, Dr. Pagnani has specific training at The Johns Hopkins Hospital in tapering individuals off of various psychiatric medications. It is unfortunately not uncommon for patients to become physiologically dependent on medications such as SSRIs, SNRIs, benzodiazepines, sleep medications and others. These medications can be incredibly helpful for some, but if a patient is continuing a medication simply because they experience withdrawal when trying to stop it, Dr. Pagnani can work with that individual to slowly taper and come off of the medication. Since this is an area of focus for Dr. Pagnani, he is also experienced in helping patients to minimize withdrawal symptoms, when a medication must be continued, or to transition patients to a new medication, with less risk for withdrawal between doses, or if a dose is missed. He can also work with compounding pharmacies to have pill doses made that allow for very small decreases is doses over-time, that cannot be achieved with the manufacturer’s pill sizes. Dr. Pagnani is experienced in tapering individuals off of Effexor (venlafaxine), Paxil (paroxetine), Xanax (alprazolam), Klonopin (clonazepam), Ambien (zolpidem), Suboxone (buprenorphine) and others.
