Are Virtual Psychiatric Appointments for Evaluations, Medication Management & Therapy Available to my Area for Anxiety, Depression, Bipolar or ADHD Treatment?
Virtual Psychiatric appointment have increased the access of mental health services throughout the country. They have also served to decrease the toll of the mental health provider shortage in many rural and other areas. Prior to offering Telehealth services for Anxiety, Depression, ADHD and other mental health struggles to patients throughout Pennsylvania, New York, New Jersey, Delaware and the other areas that we serve, patients would often drive 1-2 hours to our office in Center City Philadelphia for routine psychiatric appointments. This typically involved a day off from work, time away from family, and expenses related to travel. Now patients can take visits from the comfort of their home, or a break room during their lunch.
Providers must be licensed in both the state that they are sitting in during a virtual appointment, and the state that the patient is taking the appointment from. Therefore, Dr. Pagnani can treat patients for anxiety, depression or other conditions, who reside in Pennsylvania, New Jersey, New York, Delaware, Maryland, Florida, California and the US Virgin Islands. Policies are frequently changing, and there may be an “in-office” requirement for your initial appointment, or for follow-ups, based on your specific state. Student Mental Health, “Snow Birds” and Advantages to Telehealth Appointments for Psychiatry w/ a Provider Licensed in Several States.
If a student attends college or graduate school in a different state from where their family resides, it can cause disruptions in mental health treatment and care. For example, a student may need to wait for school breaks to return home and see their local Psychiatrist, or a student may only be able to have psychiatric appointments for their anxiety or depression treatment, when they are at school. As mentioned above, Psychiatrists who offer virtual Telehealth appointments must be licensed in both the state where they are sitting in AND the state the patient is sitting in for the visit, so if the patient is splitting time between several states, their provider may not legally be able to treat them throughout significant portions of the year.

Are there Disadvantages to Telehealth Virtual Psychiatric Appointments for Anxiety, Depression or ADHD Treatment?
There can be disadvantages to Telehealth Psychiatric appointments. Some considerations.
- Your provider may ask you to regularly see your primary care provider to have your vitals checked, if you are prescribed a medication that can affect heart rate or blood pressure. Patients may also be asked to purchase a home blood pressure cuff, and to write down data from between visits.
- Providers may not be able to receive as much information from patient body language, or may not as easily recognize signs of withdrawal, psychosis or neurological illness, when a patient is having Telehealth virtual appointments.
- Our electronic medical record, patient reminder system, video platform, and unique links for HIPAA compliant virtual appointments are typically very easy to use for most patients. However, if an individual has difficulty with technology (for example, is unable to download an app, set a phone permission to allow for microphone and camera use, or set up login information for a patient portal) then virtual psychiatric appointments would not be recommended and may not be appropriate.
- If a patient is suicidal, or in crisis, the provider may need to rely on the patient to call 911 or get immediate attention. While a provider may have the patient’s home address, they may not know exactly where the patient is, if they are taking the visit from work, a family member’s home, or another location. If a patient is seeing a provider in-office, the provider would clearly know exactly where to have crisis services arrive.
